For a long time, oral testosterone had a bad reputation and for good reason. The older formulations, methyltestosterone, were mostly processed through the liver and caused serious problems with liver toxicity. Most doctors stopped prescribing them. Injections became the standard.

Kyzatrex changed that. It's a newer FDA-approved oral testosterone undecanoate capsule that absorbs through the lymphatic system instead of the liver. That one difference is everything. It means your liver isn't getting hit with a concentrated dose of testosterone every time you take a pill. It also means the safety issues that tanked oral testosterone's reputation decades ago don't apply here.

You take it twice a day with a meal containing at least 15 grams of fat. The fat is what drives absorption through the lymphatic system, skip it and the pill doesn't work nearly as well. That's pretty much the main thing guys need to get right. Breakfast with eggs, lunch with avocado, dinner with olive oil. It fits into a normal eating pattern without much effort.

Why Older Oral Testosterone Failed — and Why Kyzatrex Doesn't

The problem with old-school oral testosterone was the route it took into your bloodstream. When you swallow most medications, they get absorbed through your gut, go straight to the liver first, and then enter circulation. For testosterone, that first pass through the liver caused real damage over time. It also meant the liver broke a lot of it down before it could do anything useful.

Kyzatrex uses a lipophilic, fat-loving formulation of testosterone undecanoate. Because it's fat-soluble, it gets absorbed into the lymphatic system through the small intestine instead of going to the liver first. The lymphatic system carries it directly into your bloodstream. Your liver is bypassed on the initial pass. The testosterone reaches circulation intact and starts doing its job.

This is the same basic mechanism used by Jatenzo, another oral testosterone undecanoate product, but Kyzatrex has a slightly different formulation and dosing profile. Both are a real step forward from what oral testosterone looked like in the past. If you've heard someone say "oral testosterone doesn't work," they were probably talking about products that predate this class of drugs by 20 or 30 years.

One thing worth knowing: Kyzatrex can raise blood pressure in some men, particularly in the first few months. We watch for this at your follow-up appointments. If it becomes an issue, we talk through whether the protocol needs to change or whether it's manageable. That's exactly why monitoring matters.

How to Take Kyzatrex Correctly

Kyzatrex is straightforward once you know the one rule that matters. Miss it and the medication underperforms. Stick to it and the protocol works the way it's supposed to.

Take With a Fat-Containing Meal

At least 15 grams of dietary fat per meal — that's roughly one egg and a slice of cheese, half an avocado, or a tablespoon of olive oil. The fat is what drives lymphatic absorption. A fasted pill or one taken with a fat-free meal will significantly reduce how much testosterone actually makes it into your bloodstream.

Twice Daily, About 12 Hours Apart

Most men take one capsule in the morning with breakfast and one in the evening with dinner. This keeps your testosterone levels more stable throughout the day compared to a single daily dose. It's a simple rhythm that most guys lock in within a week or two.

Follow-Up Labs at 6 to 8 Weeks

We recheck your testosterone, estradiol, and hematocrit at your follow-up. Kyzatrex produces peak testosterone levels 4 to 6 hours after a dose, so the timing of your blood draw matters. We'll tell you exactly when to take your pill and when to come in so the lab result reflects your actual average levels.

Is Kyzatrex actually as effective as injections?

For most men, yes. Clinical trials showed Kyzatrex brought testosterone into the normal range in the majority of patients. It won't give you the same ceiling as a high-dose injection protocol, but for someone with hypogonadism who needs their levels normalized, it does the job. If you need more precision or higher levels, Cypionate injections give you more control.

Will Kyzatrex affect my blood pressure?

It can, and it carries an FDA warning about this. The elevation is usually modest and often temporary, but we take it seriously. We check your blood pressure at every follow-up. If you already have high blood pressure that isn't well managed, we'll talk through whether Kyzatrex is the right starting point or whether another protocol makes more sense first.

Can I switch to injections or pellets later?

Absolutely. If Kyzatrex doesn't get your levels where they need to be, or if the twice-daily routine stops working for your schedule, we can move you to injections or pellets without starting from scratch. Your labs carry over and we adjust from there.

What if I forget to eat fat with my dose?

You'll absorb a lot less testosterone from that dose. It's not dangerous — but it does mean that meal basically doesn't count for your protocol. The fix is simple: build the habit. Most men pair it with breakfast and dinner and stop thinking about it after the first week or two. A small amount of fat goes a long way.

Does it affect my estrogen?

Yes, like all forms of TRT, some of the testosterone Kyzatrex delivers will convert to estrogen. We monitor your estradiol at follow-up labs. If your estrogen climbs too high and you're feeling it, we have ways to address that. It's manageable, but it needs to be watched.

Does insurance cover Kyzatrex?

It depends on your plan. Some insurance covers it with a prior authorization, others don't cover it at all because it's one of the newer branded TRT products. We'll go over your options at your first visit and be straight with you on what things cost.
